
WASHINGTON (WKOW) -- Congressional Investigators say tens of thousands of questionnaires that measure the mental and physical health of returning troops are missing.
That accounts for about 20 percent of the forms that were filled out.
The Government Accountability Board says this means the military does not have reasonable assurances that the service members got the opportunity to report their health issues on the forms.
Those questionnaires are filled out electronically and used to track health problems of troops. They give returning soldiers a chance to say whether they'd like to see a counselor.
